Referrals
Know someone who could benefit from our day centre? Referrals are welcome from individuals, families, carers, social workers, GPs, and other care professionals.
Who Can Refer?
Referrals are accepted from a wide range of sources. You do not need to be a professional to make a referral.
Individuals & Families
Individuals can self-refer, or a family member, carer, or representative can refer on their behalf with their consent.
Health Professionals
GPs, district nurses, occupational therapists, and community mental health teams can refer individuals directly.
Social Workers
Social care professionals from Staffordshire County Council or other local authorities can refer individuals to us.
Care Homes & Hospitals
Residential care homes, hospitals, and rehabilitation services can refer individuals for day care support.
Community Groups
Community organisations, befriending services, and voluntary groups can refer individuals they feel would benefit.
Other Professionals
Anyone working with an adult who has care and support needs is welcome to make a referral.
How to Make a Referral
The referral process is simple and straightforward. Here is what to expect.
Get in Touch
Call us on 0800 612 8825 or email admin@adultdaycentre.org.uk to let us know you would like to refer someone. You can also use our contact form.
Complete the Referral Form
We will send you a referral form to complete. This includes basic details about the individual, their care needs, and any relevant health or social care information.
Assessment & Visit
Once we receive the form, we will arrange an initial assessment and a centre visit. This helps us understand the individual's needs and confirm that we can provide the right support.
We aim to respond to all referrals within 3 working days of receiving the completed form.
Information We Need
To help us assess whether our centre is the right fit, please provide as much of the following as possible.
Personal Details
Name, address, date of birth, and contact information for the individual.
Care & Support Needs
A summary of the individual's care needs, mobility, and any health conditions.
Current Support
Details of any existing care arrangements, social worker involvement, or local authority funding.
Emergency Contact
Name and contact details for a family member, carer, or representative.
Relevant Reports
Any recent care assessments, GP letters, or social worker reports (if available).
Funding Information
Whether the individual has funding in place or needs help exploring funding options.
Please do not send original documents. We will request copies if needed during the assessment.
What Happens Next?
Once we receive your referral, here is the journey from first contact to first day.
We Review Your Referral
Our team reviews the referral form and any supporting information. We aim to do this within 3 working days of receiving a completed form.
Initial Conversation
We contact the individual or their representative to discuss their needs, answer questions, and arrange a convenient time for an assessment visit.
Assessment & Centre Visit
We arrange a visit to the centre so the individual and their family can look around, meet staff, and see our facilities. We complete a care needs assessment to make sure we can provide the right support.
Care Plan Agreement
If we can meet the individual's needs, we create a personalised Care Plan together with the individual, their family, and relevant healthcare professionals. This includes agreed attendance days and any specific care requirements.
Welcome to the Centre
The individual starts attending on their agreed days. We provide a warm welcome, introduce them to staff and other individuals, and make sure they feel at home from day one.
Who Is Eligible?
- Adults aged 18 and over
- Individuals with care and support needs
- Those experiencing isolation or loneliness
- People living with dementia or cognitive impairment
- Individuals with physical disabilities or mobility needs
- Those recovering from illness or hospital stays
When We May Not Be Suitable
- Individuals requiring continuous nursing care
- Those needing acute medical supervision
- People whose needs exceed our staffing levels
- Individuals whose behaviour poses a risk to others
If we cannot meet someone's needs, we will do our best to signpost them to a more suitable service.
Referral Questions
How long does the referral process take?
We aim to respond to all referrals within 3 working days. The full process, from first contact to starting at the centre, typically takes 1 to 2 weeks depending on availability and assessment scheduling.
Is there a cost for the assessment?
No. The initial assessment and centre visit are completely free of charge. There is no obligation to attend following the assessment.
Can I refer someone without their consent?
We ask that the individual is aware of and consents to the referral wherever possible. If the individual lacks the mental capacity to consent, a representative (such as a family member or legal guardian) can make the referral on their behalf in their best interests.
What if the individual is funded by the local authority?
If the individual already has a social worker or local authority funding in place, we will work directly with them to arrange attendance. If not, we can help you explore funding options. See our Funding Your Day Care page for more information.
Can someone try the centre before committing?
Yes. We encourage a trial visit as part of the assessment process. This gives the individual a chance to experience a day at the centre and meet staff and other individuals before making a decision.
